The Assembly of Canonical Orthodox Bishops of the United States of America has just published an updated map of bishops and parishes. It includes names and jurisdictions, as well as the number of parishes per county. A series of additional smaller maps give details on major urban areas, such as New York or Chicago. The document also contains an alphabetical list of all the American Orthodox bishops. Source
